Goal: To assess the impact of affordable housing on health care outcomes in a low-income population who have experienced housing instability.

Impact: Costs to health care systems were lower after people moved into affordable housing. Primary care visits went up after move-in; emergency department visits went down. Residents reported that access to care and quality of care improved after moving into housing.

Goal: The program is focused on reduction of pain and improvement of function for arthritis patients unable or unwilling to attend small group ASMPs, which have proven effective in changing health-related behaviors and improving health status measures.

Goal: The goals of HFNY are to promote positive parent-child interaction; to ensure optimal pre-natal care; to promote healthy childhood growth and development; and to enhance family functioning.

Impact: Mothers participating in the HFNY study were significantly less likely to deliver low-birth-weight babies than mothers in the control group (3.3% vs 8.3%). HFNY parents also reported having engaged in significantly fewer acts of serious abuse and neglect.

Goal: The program aims to provide comprehensive, family-centered health care for adolescent parents and their children.

Goal: The goal of Healthy IDEAS is to detect and address depression through effective, evidence-based screening and health promotion education.

Impact: Studies show that after 6 months in the Healthy IDEAS program, significantly more of the participants knew how to get help for depression (93% versus 68%), reported that increasing activity helped them feel better (89% versus 72%), and reported reduced pain (45% versus 16%) than at the beginning.

Goal: The goal of Hip-Hop to Health Jr is to reduce gains in BMI in preschool minority children.

Goal: The goals of the Holistic Health Recovery Program are to promote health and improve quality of life of injection drug users.

Impact: Implementation of the program resulted in a decrease in addition severity, a decrease in risk behavior, and significant improvement in behavioral skills and quality of life.

Goal: The goal of the Hospital Diversion Initiative is to connect individuals who chronically utilize the inpatient and emergency rooms with outpatient care.

Impact: After three months in the program, participants showed a 66% reduction in ER visits, 68% reduction in inpatient hospital stays, a 72% decrease in homelessness, an 18% reduction in unemployment, and a 66% decrease in past 30 day arrests. More than 350 individuals have been served so far.
